Most accidents caused by unsafe acts are due to failure to recognize job hazards and/or a lack of knowledge. Adequate safety training will eliminate these causes of accidents.
An Accident Report should be filled out for all accidents and near accidents, no matter how small. Accurate records of employee accidents are important for insurance claims and also for future reference and safety programs. These forms could indicate areas where the existing safety program is weak. The Plant Superintendent should complete and have on file the appropriate federal, state, and local accident reports.
